Get off the internet. If you have Google Chrome (and if you don't, download it) StayFocused is a must.
Also, really, stay off the internet and video games entirely if you're really struggling this much.

Get your parents to test you on what you're doing.

When studying, if you find yourself getting bored try putting on some quiet music, and by quiet I mean slower songs with no vocals, along with the volume
Also, never be afraid to ask the teacher if you're confused about something. (This mostly applies if you're in a smaller school, but you might still be able to do it in bigger ones)

A break always help. I would say for an hour of study, you take one hour break. Because trust me, constantly studying is only going to hurt your brain and exhaust yourself, and when there's a test or exam going on, you might actually do worse.
In relation to that, never study a day or a week before some big test, study something everyday.

Again, thank you guys for offering to help. I'm set in math, I love math. Chemistry is ok too. World History is one of my strongest this year. I start to slip up with English and Spanish. English is hit or miss with me, and this year was a total miss. I think my teacher is partialy to blame. She is unclear, confusing, easily persuaded by the class, and just SUCKS. And there was only one unit this year I actually liked and that was last unit. As for vocab... ugh. I hate that thing in it's entirety. Spanish is way too confusing at times. I actually heard somewhere that we're learning spanish wrong and using the textbooks set you BACKWARDS. Plus the vocab is even worse! I can't even understand the words much less study them...

I can do a bit with regards to English, but Spanish.... isn't my speciality. Sorry.
